I'm not feeling well. I may need to go for a major check-up next Monday.

I've been having stomach pain recently (I don't know if it's stomach pain, I haven't checked, it's just under the left rib)

The pain is really severe and unbearable (the pain makes my forehead sweat)

I want to make an appointment for a gastroscopy and colonoscopy. I may have to be hospitalized. I have to ask for leave in advance.
